The expression "see, spot, tell, smell, etc. something a mile off" means that somebody can easily recognize or identify something from a distance or very quickly because it is very obvious or distinctive. For example, if you see a very distinct-looking car on the road, you might say "I can spot that car a mile off." Or if someone is lying and you can tell that they are not telling the truth, you might say "I can smell a lie a mile off." It is often used to describe something that is very noticeable or easy to recognize.
