The expression "to cap/top it all" is used when something has already been bad or difficult, but then something else happens that makes the situation even worse. It's like the final blow that makes everything even more challenging or frustrating. It's usually used to express frustration, disappointment or anger. For example, "I had a terrible day at work, and to cap it all, I missed my train home." Another example could be, "I forgot my keys, it rained on my way to work, and to top it all, my boss was angry with me for being late."
